As Vicky states in Series 2, File 2, Item 4, “[e]ach month highlighted a differed topic related to elderly patients in acute care. Copies were printed and posted throughout hospital sites as well as collected in binders for easy reference.”

File consists of a poster which describes a regional initiative to implement the SBAR Tool, created and presented by Vicky Bach at the British Columbia Ministry of Health Innovations in Health Care Best Practices Forum in Vancouver in June 2007. The SBAR (Situation, Background, Assessment, Recommendation) Tool was developed by Vicky while employed with the Fraser Health Authority to improve nurse-physician and nurse-nurse communication in long term care facilities. File includes a one-page handout and two drafts of a small reference card, both of which were created by Vicky and outline the SBAR tool. File also includes a short video recording of Vicky reciting the introduction to an educational CD on the SBAR Tool, the contents of which she developed.

File also includes two brochures—one for patients and their families and one for staff— that she created for distribution at these facilities as part of this campaign. The brochure for patients provides “tips and resources for staying cool and safe in extreme heat conditions.” The brochure for staff provides “tips for health care providers and managers.”
