This sub-series consists of annual reports issued by the TWH Board of Governors/ Board of Trustees. The Hospital's first annual report was issued for the year 1897, but the first report surviving in the archives is the 19th, for the year 1915. This sub-series is missing reports for 1897-1914, 1919-1922, 1926-1928, 1930-1939, 1941-1943, 1948, 1959, and 1963. Reports for 1929, 1947, and 1949-1954 exist only in draft, typescript form and are incomplete.

Reports typically contain some or all of the following: brief reports by the chairman of the Board and the Superintendent/ General Superintendent/ Executive Director/ President; reports by the chairman of the Medical Advisory Committee, various medical departments, and the Director of the School of Nursing; summary financial records; lists of members of the Board, administrative staff, medical staff, and the Medical Advisory Committee; photographs of hospital buildings, facilities and staff.

This sub-series consists of annual reports issued by Grace Hospital and its predecessor, the Toronto Homeopathic Hospital. The report for the year 1899 is missing, and reports for 1900 and 1901 exist only in the form of incomplete manuscript notes. No reports after 1903 have survived.

Sub-series consists of annual reports prepared by the Superintendent/ Director of the TGH School of Nursing and submitted to the Nursing Branch, Ontario Department of Health (1930-1962/1963) and the College of Nurses of Ontario (1963/1964-1969/1970). Reports were required for accreditation as an approved school for training registered nurses in Ontario. Report for 1946 is missing.

Reports contain information about the School’s faculty (including academic credentials and professional experience), administration, students (numbers admitted, graduated, resigned etc.), curriculum (courses offered, hours of instruction provided), professional experience provided for/ required of students (hours and types of nursing service provided by students), and facilities (classrooms, laboratories, library, residence).

Sub-series consists of annual reports prepared by the Superintendent of the TGH School of Nursing and submitted to the New York State Education Department, University of the State of New York at Albany. The reports were required as a condition of the School’s continuing accreditation by the New York State Education Department.

Reports contain information about the School’s administrative and teaching staff (titles, qualifications, professional experience), students (number of students, types of student records maintained, length and cost of students’ education program, students’ professional duties), facilities, affiliations with other educational institutions, curriculum (courses offered, hours of instruction, degrees granted).

Reports for 1913, 1918 and 1924 are missing.

This sub-series consists of annual reports prepared by the TWH/Atkinson School of Nursing and submitted to:

Calendar-year reports were prepared 1931-1959, and September-August reports 1960-1973. The sub-series is missing reports for 1946 and September 1970-August 1971.

Reports include names, education and professional history, and professional responsibilities of School of Nursing administrators, faculty and staff; a statistical summary of services provided by the Hospital (number of beds, patients, and medical procedures performed); information about the School's facilities, students (number, entrance requirements, education and training duties), curriculum, and affiliation with other educational and healthcare institutions.

Some files also contain materials subsequently added, usually letters from students or their agents requesting information contained in their files.

This series consists of Dr. Bigelow's personal appointment diaries for the years 1952-1977. Two appointment books exist for each of the years 1966 and 1967: these books contain parallel but not identical information - it is necessary to consult both books for a comprehensive account of Dr. Bigelow's activities during these years. The series lacks an appointment book for 1976.

The appointment books document Dr. Bigelow's personal and professional activities, including patient consultations; surgeries performed; meetings with medical colleagues; surgical rounds; committee and professional association meetings attended; medical conferences attended; lectures and speeches delivered; dates of patients' admission to hospital and surgery, often with notes attached detailing their condition, treatment, prognosis etc.; and personal social and travel schedules.

Series consists of minutes, correspondence, historical records and photographs created or collected by the Archives and Historical Committee. Material from the Department of Medical Photography and files from the Public Relations Photo and Slide Library are interspersed within these files. Some of this may be due to the dual roles of Committee members, but in the case of files 2.8.49 to 2.8.57 they appear to be files borrowed by the Chair of the Committee. No attempt has been made to rearrange these files according to their original provenance.

File 2.8.1 includes 2 photographs of Nuclear Cardiology from the early 1980s.

File 2.8.2 contains a photograph of a construction site at TGH in July 1977 and a patient handbook from TGH from [198-?].

Files 2.8.10 to 2.8.13 were originally kept in one binder and probably belonged to Peter Edwards, who was at one point Chairman of the Historical Committee. File names are taken from the divider titles.

File 2.8.16 includes a guide for the della Robia style plaques which hung in the Outpatient Clinical in the Mulock/Larkin Wing of the Toronto General Hospital.

File 2.8.30 includes copies of photographs from the 1940s that were made in 1978.

This series consists of miscellaneous records collected and maintained by Arthur John Swanson who was General Superintendent at Toronto Western Hospital from 1930-1955. A.J. Swanson was first employed at the Toronto Western Hospital in 1925 as purchasing agent. That same year he was appointed Assistant Superintendent. In 1930 he was appointed General Superintendent, a post he held until his retirement from the Hospital in 1955. From 1956 to 1958 Swanson served as Chairman of the Ontario Hospital Services Commission. He helped found and served as President of the Ontario Hospital Association, and also served as President of the American College of Hospital Administrators. Swanson died December 5, 1960.

The official minutes for these years have not been transferred to the Archives. File 8.4.5 includes a report from the Office of the Provincial Auditor for The Toronto Hospital - Toronto General Division, 1990-1991. File 8.4.14 includes a report from the Internal Audit Department entitled “Strategic Directions, 1993/94-1995/96.” File 8.4.17 includes 1994 audit reports from the Audit Department; management responses to the audit report and an audit plan and report for 1994.

This sub-series consists of consolidated and monthly financial statements prepared for the Toronto Western Hospital Department of Finance by Eddis & Associates, Chartered Accountants. These statements and reports were distributed to members of the Board of Directors.

This series contains records concerning the involvement of the Sisters of St. Joseph of Pembroke with St. Joseph’s Hospital in Barrhead, Alberta. The records cover the establishment of the hospital, the administration and operation of the hospital, the transfer of the hospital, and the Sisters’ involvement with the Catholic Hospital Association and the Alberta Hospital Association. Materials include financial records, agreements for the purchase and sale of property, annals, memoirs, pamphlets, booklets, newsletters, lot maps, hospital floor plans, hospital bylaws, correspondence, and photographs. The major topics within the correspondence include establishing and operating the hospital, finances, the purchase and sale of property, construction and renovations, Catholic hospitals and medical ethics in Alberta, the changing role of Sisters in the hospital, and the relinquishing of ownership of the hospital. The photographs are of the hospital and Sisters at various events and locations.

Part of Dr. Donald Henry Cowan fonds

Series consists of photographs documenting the destruction of the TGH Bell Wing and the construction of the Clinical Services Building. These photographs were taken by Dr. Cowan from his offices across the street at Cancer Care Ontario (620 University Avenue, Toronto, 15th and 13th floors) from May 2001 to July 2003, in order to document the event. Eight of these pictures were used by UHN’s Public Affairs Department at the official opening of the Clinical Services Building. Photographs had been arranged by Dr. Cowan in a binder entitled <i>Toronto General Hospital Pictures Before & After</i> and divided by film number and date. These divisions are reflected in the file titles.
